Community Health Improvement

Through our Community Health Improvement office, Fletcher Allen is able to touch the lives of thousands of Vermonters each year.  Just a few examples of these programs include:

• Adult Outreach; free blood pressure and cholesterol screenings, Lifeline®, Falls and Fires home safety screenings, Diabetes Fitness Program, and Footnoters foot care program.        
• Child Passenger Safety Seat Program
• Employer Health Management
• Get Moving; physical activity program
• Hearing Outreach Program (HOP)
• Frymoyer Community Health Resource Center
• Health Ministries/Faith in Action
• Health Assistance Program; affordable heath care and medications for the underinsured or uninsured
• Poison Prevention
• Tobacco Cessation
• Vermont Blueprint for Health; improving the lives of Vermonters at risk for and living with chronic conditions, through a medical home pilot, prevention and self-management programs and provider Microsystems change.
